Securities Transfer Form

If you wish to gift financial instruments to DBU, you may transfer security(s) to the University by using the form below. This form helps notify DBU of the intended transfer of securities and provides details on how to make the transfer for you and your broker.

Instructions

1. Contact your broker to transfer security(s) to DBU’s brokerage account:
   Charles Schwab & Co. +1 (800) 435-4000
   DTC Clearing #0164, Code 40
   Account #6334-5901
   For the benefit of: Dallas Baptist University
2. Please have your broker include your name and the phrase “For the benefit of Dallas Baptist University” in the transfer instructions.
3. Complete the Securities Transfer Form below to alert DBU of your securities gift.
4. Press “Submit” at the bottom of the form to send your information to DBU.
